Contact Info Tab

• Enter the parent/guardian first name in the Parent/Guardian First Name field.

• Enter the parent/guardian last name in the Parent/Guardian Last Name field.

• Enter the parent/guardian address in the Address 1, 2, City, State and Zip Code fields.

  • Information on this form is for POS only. It does not update the Head of Household Name and Address in Manage Application.

• Enter the parent/guardian main email address in the Contact Email Address field.

• Enter the parent/guardian main phone number in the Contact Phone field.

• Click the Save button when complete.
